Methylene blue is primarily FDA-approved for treating methemoglobinemia, a blood disorder where hemoglobin cannot effectively carry oxygen, by reducing ferric iron in hemoglobin back to its functional ferrous state. It was once used as an antidote for cyanide poisoning.
It is also used off-label for various conditions, including treating vasoplegic syndrome (a type of distributive shock) to increase blood pressure, particularly when standard vasopressors are ineffective.
The dye is employed in diagnostic procedures to visualize abnormal tissues, such as identifying cancerous lymph nodes during sentinel node dissections, highlighting dysplastic lesions in the gastrointestinal tract during chromoendoscopy, and aiding in surgical procedures like bowel leak testing or bone cement differentiation.
Additionally, methylene blue has been used to treat complications like ifosfamide-induced encephalopathy, and it has shown potential in photodynamic therapy for nail fungus and as a possible cognitive enhancer due to its effects on mitochondrial function, though these uses remain under investigation.
